Upper School Planned Absence Form

If a student knows in advance that they will miss all or part of a school day, they are expected to complete a Planned Absence Form, in addition to a parent completing the Student Logistics Request Form.

This Planned Absence Form must be signed by all teachers and the student’s advisor before being submitted to Mrs. Kasserman in the Upper School Office.

If the absence will be more than three days, or if the student has already missed 5 class periods for a semester course (or 10 for a full-year course), they must also obtain a signature from Mrs. Aviles, the Academic Dean.
